Please keep in mind that I am an addictions counselor, and I work in a methadone program. Many of the
patients are in the program for an indefinite period of time. A large amount of the patients have been in
the program for several years.
60 year old Hispanic male, who has a history of heroin use disorder. Reports PTSD and depression issues.
He has a plethora of medical issues such as chronic arthritis, high blood pressure, high cholesterol and
diabetes. He continues to struggle with his addiction, as he uses heroin on a daily basis. He currently is
receiving SSI, however he is currently homeless and looking for housing. This client appears to have a
desire to make some changes in his life, however he lacks motivation to change the direction of his life.
Patient reports having many stressors, and uses the fact that he has stressors as an excuse to use illicit
substances. When I first met this client I had difficulties in building a rapport with him because he didn’t
trust me enough to be forthcoming. In previous sessions he wouldn’t be forthcoming about his illicit
substance use, however when I reviewed the toxicology report it revealed that he was using heroin and
alcohol.
I genuinely have the client’s best interest at heart, and trying my best to meet the client where he is, as
it relates to his addiction. I’ve always been the type of individual who deals with reality without numbing
my emotions with mind altering drugs. Interacting with an individual who mask his problems with the
use of illicit substances is somewhat frustrating. Sometimes I find it hard to empathize with this patient
because he doesn’t utilize other alternatives, as it relates to coping with stress.

Worker-Hey how are you doing?
Patient-I’m ok I guess but I have a lot of problems that I’m going through
Worker- Is Everything ok?
Patient- No because I had to testify against the man who stabbed me.
Worker- Wow that must have been a very traumatic experience, how did prepare yourself for the
testimony?
Patient- Let’s just say I had to do what it takes to feel comfortable. I didn’t want to do it because I’m
from the streets where snitches get stitches.
During this session I used open ended questions, probing, exploring and expressing empathy
throughout the session.
