Pennsylvania has been swept with deaths related to overdose since the early 2000s. York County
is ranked 2nd in the entire state for overdose related deaths. This epidemic is not restricted to a
certain area, rural and urban areas are equally affected (Assess).
The ultimate purpose is to construct a multidisciplinary force that consists of the
community, medical professionals, and government agencies that commit to reducing substance
abuse (Mobilize). By the end of 2016 York County was faced with 180 substance abuse related
deaths and 365 reversed overdoses (Track) Drug-related deaths in York County go up for 2016.
(2017, March 07). Education, awareness, and reinforcement is the key to helping individuals
understand the affect substance abuse and the multiple areas of your life it destroys; mentally,
physically, social life, family, employment, and how you function in society (Implement). A
community centered approach aimed at improving the quality of health of the population will
reduce the number of substance abuse, which in turn will decrease the number of substance
abuse related deaths (Plan). A force of government agencies, law enforcement, the community,
and medical professionals will be needed to complete the mission.

Risk Assessment
The Burn’s family have been residents of York County for over 55 years. The family consists of
three people, in which is the grandmother Susie (83 years of age) with a seizure disorder, chronic back
and hip, along with a history of multiple TIA’s.The grandfather Julian (85 years of age) has hypertension,
high cholesterol, and multiple herniated discs from an on the job accident while working construction.
The grandson Anthony (29 years of age) is an abled body male with no medical history. Little to no
family interaction exists in the home. The Burns are a low class family that lives in a subsidized housing
projects. Both Susie and Julian receive SSI and Anthony is currently unemployed. They still currently
follow Catholic cultural practices. The highest level of education is the 10th grade between the three of
them.
The grandparents adopted the grandson when he was the age of 4 due to the biological mother
having an opioid addiction. The biological mother later died three years later from what was presumed
an overdose. The grandparents did not provide any structure for Julian growing up out of guilt from his
mom giving him up for her addiction. A lot of Julian’s negative actions were overlooked and many
excuses were made for this reason. The Grandparents are regretful for not providing Julian with any
structure as evidence by the sad look on their faces as they recall the past.
Julian is currently stealing the grandparents pain medications for IV drug usage and
“occasionally sharing with friends,” states Julian. The reason reported by Julian for IV drug usage was, “I
just do it to escape my depression and also so I’m accepted, that way no one will mess with my
grandparents.” The grandparents are currently aware of Julian’s substance abuse and form of receiving

the drugs, but are unable to do anything about it. The crime/violence, drug, and alcohol rate is high. The
community refuses to get involved because they are afraid of the outcome and do not feel safe.
There is no personal transportation and due to having no other living family members that they
know of, the neighbors are relied on for grocery story runs, appointments and anything else that
involves getting out the house. The grandfather just feels it is best to ignore the family issues in fear of
putting stress on each other’s health conditions.

The risk assessment results suggest that the quality of life in the Burns family is poor. Their living
environment has a high crime, violence, drug and alcohol rate that restricts the grandparent’s
independent life functions from fear of getting into a life threatening situation. The grandson Julian
is at most risk for extensive psychological, physical issues, and an increased risk of death due to IV
drug usage, no identified coping skills, and the grandparents lack of resources to take legal action for
Julian’s drug abuse and stealing narcotics. The grandparents are at high risk due to having a limited

support system, dangerous environment, but are able to identify one cultural protective factor by
believing and following Catholic practices.
