MULTIDISCIPLINARY APPROACH DEFINITION
Jon Conklin is a licensed medical social worker employed by Riverbend City Medical Center (RCMC).
Hired by RCMC immediately after graduating from college, Jon wants to make an impression on his
colleagues as an independent self-starter. He doesn’t believe in asking for help or seeking assistance
with his cases, mostly because he thinks others will see that as a weakness. As a result, he is particularly
challenged in working with Celia.
In this exercise, you will help Jon make decisions that will affect Celia’s care.

Celia is receiving treatment for breast cancer. She is 39 years old and has two children, ages 6 and 4.
Her husband has a minimum wage job at a meat processing plant and has a commute of over an hour
each way. Celia and her family are enrolled in the state health care program.
When Jon was assigned Celia’s case, the first issue she was facing was coordinating child care with her
chemotherapy schedule. She needs to come in once every two weeks and sessions could last anywhere
from 3 to 6 hours. While explaining her circumstances to Jon, Celia breaks down in tears and tells him
that she can’t afford a baby sitter and the preschool will only keep her 4 year old in the mornings.
Option 1
“We could have a volunteer sit with your children while you are receiving your treatment.”
On the first day that the volunteer is supposed to sit with Celia’s children, a miscommunication is
discovered. The volunteer assigned is only able to stay for two hours. She calls Jon, who ends up going
down to the clinic and sitting with Celia’s children. His supervisor talks to him later, telling him that he
cannot be unavailable for half a day and another solution has to be found.
Option 2
“There is a drop-in child care center in the hospital. We should be able to arrange for you to use that.”
Celia brings her younger child to the drop-in center. The center is not aware of any special pricing and
Celia has to pay over $30 for the child care – money she can’t spare. The next time she comes for
chemotherapy, she brings her child to the clinic. While she has brought books and toys, the clinic
supervisor contacts Jon and tells him he needs to make sure Celia understands that she cannot do that
again.

The chemotherapy clinic contacts Jon and tells him that Celia has been late for her last two appointments
and needs help with transportation issues. When Jon contacts her, Celia tells him that her husband has
been assigned to a different shift. Previously, he had been dropping them at RCMC on his way to work,
but he now has to be at work by 7:00 am she just can’t get herself and the kids ready early enough for
him to get to work on time.
Option 1
“It’s important that you find a way to get here on time. I take the bus to work and I could help you figure
out how to get from your neighborhood to the hospital.”
Celia works with Jon and does her best with busses, but is late or altogether misses several
chemotherapy sessions. At her second to last session, one of the oncology nurses asks her why she
hasn’t been using taxi vouchers from the state health and human services office. Celia calls Jon, who tells
her that he didn’t know the state offered those vouchers.
Option 2
“Let’s brainstorm some solutions. Maybe a friend or family member would be able to drive you here on
your appointment days.”
Celia reluctantly asks her mother-in-law for rides to her appointments. Her mother-in-law agrees, but
complains about the inconvenience and is unreliable. The nurse supervisor of the chemotherapy clinic
contacts Jon and tells him that Celia’s stress is increasing and could harm her health.

Celia has completed chemotherapy, but is now supposed to receive radiation therapy. This will entail
coming into the clinic Monday through Friday for the next six weeks. The sessions are short, but the
scheduling is tight and Celia will need to be on time every time.
Celia meets with Jon and is very discouraged. During their meeting she suggests that she might just stop
all treatment at this point in hopes that the chemotherapy will have been enough. She tells Jon that she
appreciates what he’s tried to do for her, but it has been frustrating to find out after the fact that there
were other resources.
Option 1
“I can understand why you’re frustrated, but there are so many resources out there. I know about a lot,
but not all of them.”
Celia leaves Jon’s office saying that she will think about the radiation treatment, but he is certain that she
will not do it. He feels sick at heart because he believes he has failed her.

“Let’s try something different. I’m going to put together a team of people from different disciplines here in
the hospital. Maybe that way we can get the answers and resources you need when you need them.”
When Jon reaches out to his supervisor and peers and begins to take a more multidisciplinary approach,
he not only is better able to respond to Celia’s needs, he learns that he is better able to do his job for all
his clients.
His supervisor gives him positive feedback when he engages other resource people.
Jon realizes that he has made an important discovery in terms of his professional practice.

professional’s initial proposed problem or another problem based on information
shared within the group.
